The Regional Office of Undergraduate and Graduate Medical Education (UME/GME) at Kaiser Permanente Northern California (KP) is your educational partner. We are committed to creating and sustaining a safe and inclusive clinical learning environment which supports your concerns and feedback.
Use this form for reporting confidential concerns. You are not required to provide your name or any other identifying information. However, if you request a follow-up you may complete your contact information at the end of the form.
Reporting concerns or giving feedback takes courage. Sharing your feedback with us will not only help you address your area of concern, it can also help strengthen the learning environment overall. The GME Director/DIO and Medical Director will review your report to determine the appropriate next steps based on the type of report and details in the form. Reports of alleged mistreatment and negative learning experiences reported through the feedback form will be reviewed and handled in a timely manner.
Protection from Retaliation
Retaliation against or intimidation of any individual who seeks advice, raises a concern, or reports unprofessional misconduct or mistreatment in good faith will not be tolerated. Anyone who deliberately makes a false accusation with the purpose of harming or retaliating against another member of the KP UME/GME community will be investigated and appropriately addressed.
